L. 104-13) and 5 CFR 1320.5 (a) (i) (iv), this notice announces that OPM has submitted to the Office of Management and Budget (OMB) a request for clearance of an information collection. [[Page 13293]] The Declaration for Federal Employment, Optional Form 306, is used by OPM and other Federal agencies to collect information to determine the individual's acceptability for Federal employment and enrollment status in the Government's Life Insurance program.
